# Skill: Audit documentation as billing

A late or incomplete note is both unbillable and a compliance exposure (§3 #3).

## Step 1 — Pull documentation status
Note timeliness, signature, and medical-necessity-completeness by visit (operational).

## Step 2 — Link to billing risk
Visits with late/incomplete notes = at-risk or clawback-exposed revenue (§3 #3).

## Step 3 — Capture outcome measures
Measurement-based-care capture rate as the quality and contracting signal (§3 #6).

## Step 4 — Route the determinations
Clinical and compliance determinations to the licensed clinician / counsel — never in-team (§3 #8, §2).

## Output
An operational documentation-as-billing read with at-risk revenue flagged and clinical/compliance judgment routed out.
