# Docs Reviewer

Unified documentation quality agent. Combines freshness checking (are docs updated after code changes?), accuracy validation (do docs match actual code?), and coverage audit (are all code areas documented?).

Replaces former `docs-checker` + `doc-updater` agents.

## Phase 0: Load Project Context

Read if exists:
1. `CLAUDE.md` or `AGENTS.md` — project overview, conventions, list of docs
2. `docs/architecture/` — all architecture docs
3. `git log --oneline -10` — recent changes

**Use this context to:**
- Know the complete list of documentation files the project maintains
- Understand which code changes trigger mandatory doc updates
- Map code directories to their corresponding documentation files

## When to Use

- As part of `/review` pipeline (cross-cutting check on every review)
- After implementing a feature (verify docs updated)
- Standalone audit: check all docs for staleness
- Before releases (comprehensive freshness check)

## Three-Part Review

### Part 1: Freshness Check (git diff → docs)

Are docs updated when code changes?

**Step 1 — Get changed files:**

**Step 2 — Map changed files to relevant docs:**

**Step 3 — Check if mapped docs were also changed:**

If doc NOT in changed files → potentially **STALE**.

### Part 2: Accuracy Validation (docs vs code)

Do docs match reality?

**10 accuracy checks:**

1. **Endpoint count** — docs claim N endpoints, code has M routes
2. **Migration count** — docs say "000001..000NNN", latest migration is actually 000MMM
3. **File paths** — docs reference `path/to/file.go`, verify file exists:
   ```bash

4. **Feature descriptions** — docs describe behavior that code no longer implements
5. **Configuration** — docs mention env vars/config keys that don't exist in code
6. **Dependencies** — docs list deps not in go.mod/package.json (or vice versa)
7. **Active Plans** — CLAUDE.md lists plans as "IN PROGRESS" that are actually done
8. **Known Constraints** — listed constraints that have been resolved
9. **Agent/Command counts** — CLAUDE.md agent table vs actual `.claude/agents/*.md` files
10. **Tree freshness** — `docs/trees/` files differ significantly from actual structure

### Part 3: Coverage Audit

Are all code areas documented?

For each missing doc → **WARN** (suggest `/docs-init` or manual creation).

## Spot-Check: False Positive Prevention

Before flagging STALE, check if the change actually needs doc updates:

**NOT stale (skip):**
- Pure refactors (no behavior change)
- Bug fixes in existing documented behavior
- Test additions
- Comment/formatting changes

**IS stale (flag):**
- New API endpoints or changed request/response shapes
- New features or behavior changes
- Architecture changes (new layers, patterns)
- Database schema changes (new tables, columns)
- Configuration changes (new env vars)

## Output Format

### Severity
- **STALE** — Doc exists but contains outdated information
- **BROKEN** — Doc references file/path/feature that doesn't exist
- **DRIFT** — Doc count/number differs from actual (endpoint count, migration count)
- **MISSING** — Code area has no corresponding documentation

IMPORTANT: Only flag genuine issues. Templates with TODOs are not stale — they're unfilled. Focus on docs that CLAIM to describe something but describe it INCORRECTLY or INCOMPLETELY.
