# Managing Blood Bank Compatibility

Guides blood product compatibility testing with antibody identification and crossmatch protocols.

## Why This Skill Exists

Transfusion medicine errors are among the most dangerous events in laboratory medicine. An ABO-incompatible red cell transfusion can cause fatal acute hemolytic transfusion reaction (AHTR) with mortality rates of 10-40%. The FDA mandates reporting of transfusion-related fatalities, and AABB Standards for Blood Banks and Transfusion Services (34th edition) set the minimum requirements for pretransfusion testing, antibody identification, and product selection. CAP accreditation under the Transfusion Medicine (TRM) checklist series requires documented procedures for every step from sample collection to product issuance.

CLIA regulations and state licensure requirements demand that blood bank personnel demonstrate competency in antibody identification, crossmatch procedures, and emergency release protocols. The complexity of alloantibody workups — multiple antibodies, antibodies to high-frequency antigens, autoantibodies masking alloantibodies — requires systematic approaches that this skill codifies. A structured process prevents the most dangerous class of laboratory errors: wrong blood to wrong patient.

## Checkpoint A: Pre-Draft Intake (Mandatory)

1. **Clinical scenario** — Routine pre-surgical, active bleeding, massive transfusion, prenatal, chronic transfusion patient, or transplant? Default: routine pre-surgical.
2. **Patient history** — Prior transfusion reactions, known antibodies, previous pregnancies, transplant status? Default: no history; flag [VERIFY].
3. **Current sample** — Specimen type (EDTA tube), time of collection, two-identifier verification performed? Default: EDTA, < 72 hours.
4. **Urgency** — Routine, urgent (1-2 hours), emergent (immediate release)? Default: routine.
5. **Product type needed** — RBCs, platelets, plasma, cryoprecipitate, or granulocytes? Default: RBCs.
6. **Special requirements** — Irradiated, CMV-negative, HbS-negative, washed, volume-reduced, extended antigen-matched? Default: none.
7. **Number of units** — How many units anticipated? Default: 2 units RBC type and screen.

## Step 1: ABO/Rh Typing and Confirmation

Perform ABO and Rh(D) typing following AABB Standards:

- **Discrepancy resolution**: If forward and reverse do not agree, hold and investigate: cold autoantibodies, missing antibodies (hypogammaglobulinemia, age < 4 months), subgroups (A2 with anti-A1), or recent transfusion of non-type-specific products.
- **Rh(D) weak D testing**: Per AABB/CAP recommendations, test for weak D in donors (always) and patients (institutional policy; recent guidance supports genotyping for weak D types 1, 2, 3 which can safely receive Rh-positive blood).

## Step 2: Antibody Screening

Perform antibody screen using FDA-licensed screening cells (2-3 cell panel) per AABB Standards:

- **Methods**: Immediate spin (IS), 37°C incubation, indirect antiglobulin test (IAT) using PEG, LISS, or solid-phase technology.
- **Interpretation**:
  - All cells negative at all phases: Screen negative, proceed to electronic crossmatch (if criteria met) or serologic crossmatch.
  - One or more cells reactive: Proceed to antibody identification panel.

**Electronic crossmatch eligibility** (per AABB Standards 5.16.3):
- Two concordant ABO/Rh typings on file (current and historical)
- Negative antibody screen on current specimen
- Validated computer system with logic to prevent ABO-incompatible issuance

## Step 3: Antibody Identification

When the antibody screen is positive, perform a full panel workup:

1. **Panel testing**: Run an 11-16 cell identification panel at IS, 37°C, and IAT phases.
2. **Rule-out process**: Eliminate antigens present on non-reactive cells. An antibody can be ruled out when at least two panel cells homozygous for the antigen are non-reactive (per rule of three for statistical validity).
3. **Common clinically significant alloantibodies**:

4. **Additional techniques for complex cases**:
   - Enzyme treatment (ficin, papain): Enhances Rh, Kidd, Lewis; destroys Duffy, MNS
   - Adsorption (auto or allo): Warm autoantibody removal to unmask underlying alloantibodies
   - Elution: Identify antibody specificity from coated red cells
   - DTT treatment: Denature IgM antibodies or Kell antigens

## Step 4: Crossmatch and Product Selection

Select compatible products based on antibody identification:

- **Antigen-negative unit selection**: Screen donor units for absence of the corresponding antigen. Confirm antigen-negative status before issuing.
- **Crossmatch**: Perform serologic crossmatch (IAT) for patients with clinically significant antibodies. Electronic crossmatch permitted only when screen is negative and eligibility criteria are met.
- **Extended antigen matching**: For chronically transfused patients (sickle cell disease, thalassemia), match for C, E, and K at minimum per AABB recommendations. Consider extended Rh and Kell matching (C, c, E, e, K) prophylactically.
- **Emergency release**: When life-threatening bleeding cannot wait for crossmatch, issue uncrossmatched O-negative (or O-positive for males) RBCs. Document emergency release per institutional protocol. Switch to type-specific as soon as possible.

## Step 5: Documentation and Release

- Record all testing results, interpretations, and product selection rationale.
- Tag each unit with patient identifiers, ABO/Rh compatibility statement, special requirements, and expiration.
- Final bedside verification: Two-identifier patient check, product label verification, and ABO compatibility confirmation immediately prior to transfusion.
- Document time of release, releasing technologist identity, and receiving nurse/provider.
- For massive transfusion protocol (MTP) activation, document MTP start time, product ratios issued (target 1:1:1 RBC:FFP:platelet), and communication log with clinical team.

## Checkpoint B: Post-Draft Alignment (Mandatory)

1. Are ABO forward and reverse typing concordant, and if not, is the discrepancy investigation documented?
2. Is the antibody identification supported by rule-out of at least two homozygous-positive non-reactive cells per antibody specificity?
3. Are antigen-negative units confirmed negative before crossmatch?
4. Is the crossmatch method appropriate for the patient's antibody status (electronic vs. serologic IAT)?
5. Is emergency release documented with proper rationale and follow-up completion plan?

## Guidelines

- Never issue a blood product without two-identifier patient verification at both the testing and release stages
- Treat all Kidd system antibodies with heightened vigilance — anti-Jka and anti-Jkb are notorious for evanescent titers and causing delayed hemolytic transfusion reactions
- For patients on anti-CD38 therapy (daratumumab), use DTT-treated screening cells or genotyping to avoid pan-reactivity interference
- Always rule out underlying alloantibodies when a warm autoantibody is present; adsorption studies are mandatory before antigen-negative unit selection
- Maintain a crossmatch-to-transfusion ratio (C:T ratio) below 2.5:1 per AABB best practice to minimize wastage
- Document all emergency uncrossmatched releases and complete full compatibility testing retrospectively within 24 hours
- For prenatal patients, perform antibody titers at defined intervals (every 4 weeks until 24 weeks, then every 2 weeks) for clinically significant antibodies
- Participate in AABB-required proficiency testing and maintain inter-laboratory concordance for antibody identification
