# Managing Payer-Specific Coding

Adapts coding practices for payer-specific requirements across Medicare Fee-for-Service, Medicare Advantage, Medicaid (state-specific), TRICARE, and commercial payers. Covers LCD/NCD compliance, payer-specific modifier rules, coverage determination navigation, prior authorization coding requirements, and payer contract analysis for coding impact.

## Why This Skill Exists

No single coding rule set applies to all payers. Medicare FFS follows CMS rules; Medicare Advantage plans may add supplemental coverage or restrict services differently; Medicaid varies by state; commercial payers follow their own medical policies, bundling edits, and modifier requirements. Coding to the "wrong" payer's rules is the root cause of 20–30% of claim denials. Key differences include: consultation code recognition (Medicare does not recognize them; most commercial payers do), telehealth modifier requirements, modifier 59 vs. X-modifier acceptance, bilateral procedure reporting, and coverage for specific services. A payer-aware coding process eliminates preventable denials.

## Step 1 — Medicare FFS-Specific Coding Rules

Apply CMS-specific rules that differ from general CPT/ICD guidelines.

**Key Medicare-specific rules:**
- **Consultation codes (99241–99255)**: Medicare FFS does NOT recognize consultation codes. Office consultations must be billed as new or established patient E/M codes (99202–99215). Inpatient consultations must be billed as initial hospital care (99221–99223) with modifier AI (principal physician of record).
- **Bilateral procedures**: Medicare requires modifier 50 on a single line with 1 unit. Do NOT report two lines with RT/LT unless the specific MAC or code requires it.
- **NCCI edits**: Medicare applies full NCCI PTP and MUE edits. X-modifiers (XE, XS, XP, XU) are preferred over modifier 59.
- **Incident-to billing**: Services performed by NPPs under direct supervision of a physician can be billed under the physician's NPI with no reduction. Requires: established patient, ongoing course of treatment, physician present in the office suite.
- **Teaching physician rules**: The teaching physician must be present during the key/critical portions of the service. They must document personal involvement. Modifier GC indicates a service performed by a resident under the teaching physician's direction.
- **Global surgical periods**: 000, 010, and 090-day global periods apply strictly. The CMS Physician Fee Schedule assigns the global period for each CPT code.
- **Advance Beneficiary Notice (ABN)**: Required before providing a service Medicare may not cover. The ABN shifts financial responsibility to the patient. Services billed with modifier GA (ABN on file) or GX (voluntary ABN, not required) or GZ (expected denial, no ABN on file — provider accepts liability).

## Step 2 — Medicare Advantage-Specific Rules

Understand where MA plans follow CMS rules and where they diverge.

- MA plans must cover everything Original Medicare covers but may have additional benefits and different utilization management.
- **Prior authorization**: MA plans frequently require prior authorization for services that Medicare FFS does not (surgeries, advanced imaging, specialty referrals, DME).
- **Consultation codes**: Some MA plans recognize consultation codes (99241–99255) even though Medicare FFS does not. Check the plan's provider manual.
- **Supplemental benefits**: MA plans may cover services not covered by Medicare FFS (dental, vision, hearing, OTC benefits, telehealth expansions).
- **Value-based care modifiers**: Some MA plans require quality reporting modifiers or value-based payment indicators.
- **Risk adjustment**: MA plans require HCC-eligible diagnosis codes for risk adjustment (see Managing Risk Adjustment Coding skill). This creates a coding emphasis on chronic condition specificity.
- **Network requirements**: MA plans may deny claims for out-of-network services except in emergency/urgent situations.

## Step 3 — Medicaid-Specific Coding Rules

Apply state-specific Medicaid rules — each state is different.

**Common Medicaid variations:**
- **Fee schedules**: Medicaid reimbursement rates are typically 60–80% of Medicare rates but vary dramatically by state.
- **Covered services**: Each state defines its own covered services list. Some states cover services Medicare does not (e.g., adult dental, vision).
- **Modifier requirements**: Many states require specific modifiers for behavioral health services (HO for master's-level providers, HN for bachelor's-level, HQ for group settings).
- **Prior authorization**: Medicaid managed care plans have their own prior authorization requirements separate from the state fee-for-service program.
- **EPSDT (Early and Periodic Screening, Diagnostic and Treatment)**: For Medicaid patients under 21, EPSDT requires coverage of all medically necessary services even if the service is not on the state's adult covered services list.
- **Timely filing**: Medicaid timely filing deadlines vary by state (90 days to 365 days from date of service).
- **Dual-eligible patients**: For patients with both Medicare and Medicaid, Medicare is primary and Medicaid is secondary. Medicaid may cover the Medicare deductible and coinsurance (crossover claims).

## Step 4 — Commercial Payer-Specific Rules

Navigate private payer variations that differ from Medicare.

**Key commercial payer variations:**
- **Consultation codes**: Most commercial payers DO recognize consultation codes (99241–99255). Billing office E/M codes instead of consultation codes when a consultation was performed may result in lower reimbursement.
- **Bilateral procedures**: Many commercial payers require two separate lines with RT and LT modifiers rather than modifier 50 on a single line. Check the payer's claim submission guidelines.
- **Modifier 59 vs. X-modifiers**: Some commercial payers do NOT recognize X-modifiers (XE, XS, XP, XU) and require modifier 59. Others follow Medicare's X-modifier preference. Using the wrong modifier for the payer results in a denial.
- **Bundling edits**: Commercial payers may use proprietary bundling software (e.g., McKesson ClaimsXten, Optum) with edits that differ from NCCI. A code pair that passes NCCI edits may still be denied by a commercial payer's proprietary edits.
- **Out-of-network billing**: Balance billing restrictions vary by state. Some states prohibit balance billing for emergency services; others have broader surprise billing protections under the No Surprises Act.
- **Medical policies**: Each payer publishes its own medical policies — these function like LCDs/NCDs but are specific to the payer. They define covered diagnoses, frequency limits, age restrictions, and documentation requirements for specific services.
- **Timely filing**: Varies from 90 days to 1 year depending on the payer contract.

## Step 5 — LCD/NCD Coverage Navigation

Validate service coverage before or during claim submission.

**National Coverage Determinations (NCDs):**
- NCDs apply to ALL Medicare FFS claims nationwide. They are published by CMS and supersede LCDs.
- NCDs define whether a service is covered, under what conditions, and with what limitations.
- Key NCDs: NCD 220.6 (PET scans for cancer), NCD 190.34 (screening PAP smears), NCD 210.7 (sleep testing), NCD 220.1 (CT colonography).

**Local Coverage Determinations (LCDs):**
- LCDs are published by MACs and apply to specific jurisdictions (A/B MAC jurisdictions).
- LCDs define which ICD-10-CM codes establish medical necessity for a covered service.
- Billing a service without a covered diagnosis code results in a medical necessity denial.
- LCDs are accompanied by LCD-related Local Coverage Articles (LCAs) that provide additional coding guidance.
- Use the CMS Medicare Coverage Database to look up applicable LCDs for the provider's jurisdiction.

**Coverage verification workflow:**
1. Identify the CPT/HCPCS code being billed.
2. Check for an applicable NCD. If covered nationally, LCD is supplementary.
3. If no NCD exists, check the LCD for the MAC jurisdiction.
4. Verify the ICD-10-CM diagnosis meets the LCD's covered diagnosis list.
5. If the diagnosis is not on the LCD's covered list, determine if an ABN is needed (Medicare) or if the service will be denied.
6. Document the coverage determination in the billing work queue.

## Step 6 — Manage Prior Authorization Coding Requirements

Ensure coded claims match authorized services.

- The CPT/HCPCS codes on the claim must match the codes authorized in the prior authorization.
- If the performed procedure differs from the authorized procedure (e.g., intraoperative findings required a different approach), document the change and notify the payer.
- Some payers require specific diagnosis codes to match the authorization — a different diagnosis code may trigger a denial even if the procedure code matches.
- Retroactive authorization may be available for emergent services — check the payer's policy and submit the retroactive request within the required timeframe.
- Track authorization expiration dates — authorizations have limited validity periods (typically 30–90 days from approval).
- For inpatient admissions, verify the authorized number of days matches the actual length of stay. Concurrent reviews may extend the authorization during the stay.
