## FAILURE MODES

### Schema Bloat
**Symptoms**: Decomposition produces >15 nodes for simple problems, excessive nesting depth >4 levels
**Diagnosis**: P×C stopping rule threshold too low, domain meta-skill overly granular
**Fix**: Increase threshold by 0.05, validate leaf nodes are single-skill achievable

### Rubber Stamp Review
**Symptoms**: All nodes assigned COMMITTED level, no vague nodes despite uncertain dependencies
**Detection Rule**: If commitment_distribution shows >80% COMMITTED in problems with future unknowns
**Fix**: Force TENTATIVE for nodes depending on >2 other nodes, create vague nodes for uncertain work

### Cascade Depth Explosion
**Symptoms**: Single node failure cascades to >6 downstream nodes, wave planning serial despite parallel potential
**Diagnosis**: Failure domains not properly identified, excessive coupling in decomposition
**Fix**: Split high-cascade nodes into smaller units, identify shared dependencies as separate failure domains

### Incomplete Skill Matches
**Symptoms**: Nodes assigned skills whose output schemas don't cover all required fields
**Detection Rule**: If skill.output_schema missing >20% of node's required outputs
**Fix**: Split node into multiple concrete nodes, or create vague node for unmatched portion

### Wave Planning Deadlock
**Symptoms**: Circular wave dependencies, nodes with no valid wave assignment
**Diagnosis**: DAG has cycles or failure domain constraints over-restrict scheduling
**Fix**: Break cycles by splitting nodes, relax failure domain isolation for low-risk shared dependencies

## WORKED EXAMPLES

### Example 1: Simple DAG - Code Refactoring
**Input**: "Refactor UserService class using dependency injection"
**Domain**: software-engineering, **Budget**: STANDARD

**Pass 1 Decisions**:
- Load `meta-software-engineering` → suggests functional decomposition
- Node 1: "Analyze current UserService" (concrete, confidence 0.9)
- Node 2: "Design DI patterns" (concrete, confidence 0.8) 
- Node 3: "Apply refactoring" (vague, depends on 1+2, confidence 0.6)

**Pass 2 Decisions**:
- Node 1: `code-review` skill (Step 4: pattern recognized, conf 0.85)
- Node 2: `refactoring-surgeon` skill (Step 3: domain match first)
- Node 3: Remains vague (will resolve in Wave 1 planning)

**Pass 3 Decisions**:
- Wave 0: [Node 1] (no deps)
- Wave 1: [Node 2] + resolve Node 3 → splits into 3a, 3b, 3c
- Commitment: COMMITTED for 1,2 (high confidence), TENTATIVE for resolved 3x

### Example 2: Vague Node Resolution
**Scenario**: Node 3 from Example 1 gets resolved after Wave 0 completes
**Wave 0 Results**: Analysis reveals 3 distinct refactoring areas

**Resolution Process**:

### Example 3: Skill Mismatch Recovery
**Problem**: Node assigned `test-writer` but needs integration testing, not unit testing
**Detection**: Post-assignment validation catches output schema mismatch

**Recovery Process**:
1. **Symptom**: `test-writer` outputs unit test files, node needs integration test suite
2. **Diagnosis**: Step 1 signature filter failed, missed integration vs unit distinction
3. **Fix**: Re-run cascade with refined context conditions, assigns `fullstack-debugger`
4. **Prevention**: Update skill library with more specific integration testing skills

## QUALITY GATES

- [ ] All three passes executed in sequential order (no skips, no reordering)
- [ ] Pass 2 made zero LLM calls (deterministic skill selection only)
- [ ] Every vague node has only role_description and dependency_list fields
- [ ] No vague node has skill_assignment, agent_config, or model_selection fields
- [ ] Wave assignments respect topological order (no node before its dependencies)
- [ ] No nodes sharing failure domains are in the same wave
- [ ] Deferred waves marked with planned: false
- [ ] DAG passes acyclicity check (topological sort succeeds)
- [ ] Every concrete node has a skill assignment from the cascade
- [ ] Commitment levels respect deliberation budget constraints
- [ ] Cascade depth score computed and logged
- [ ] All four decomposition log fields present (meta_skill, method, commitment_distribution, cascade_depth)
