End-to-end feature spec workflow. Outputs a domain doc (if new) and a GitHub Epic whose body is the drafty spec. Sub-stories are filed as linked sub-issues. The spec remains live and editable in the Epic body until /pstack-execute freezes it into docs/specs/<name>.md on the first story.
