Set up a known-issues backlog in any project. An entry is filed only when the user asks for one, with a dated, immutable id in docs/TODO.md. Closing stays automatic: every substantive session sweeps the backlog for entries its own work resolved and closes them on evidence. A policy in the agent-instructions file holds future sessions to both rules. The point is that a defect outlives the session that found it, when the user decides to keep it.
